Vision screening catch-up clinic enquiry form

Our team run vision screening clinics during school holidays at community locations, so all Prep-aged children can get their eyes checked during their Prep year. If your child lives in Queensland and missed getting their vision screened at school or if your child is home-schooled, you can find out about attending one of our catch-up clinics.

By submitting this form, you consent to our team contacting you to discuss the Primary School Nurse Health Readiness Program (PSNHRP) vision screening catch-up clinics. We will call you from a private number, within 2 business days of receiving your enquiry.
