Health Provider Portal
You can view health care records for your patient through the Health Provider Portal (HPP) website. If you don't already have access to the portal, you'll need to register.
Records include:
- pathology reports
- discharge summaries
- medical imaging reports
- medication list
- outpatient letters
- operation reports
- test results.
My Health Record
You may be able to access patient records if your patient has a My Health Record on the digitalhealth.gov.au website.
Health Information Access service
You can ask to access patient records through our Health Information Access service.
Your request should be in writing and on your practice letterhead to ensure compliance with the Hospital and Health Boards Act 2011.
Email your request to CHQ_HIA@health.qld.gov.au.
We usually give access to records in 48 hours.
