Personal health record (red book)

The Personal Health Record (PHR) is a free booklet provided to the parent/s of every child born in Queensland. The parent-held booklet is used to record your child’s immunisations, developmental checks and other major health events. It is important to keep your child’s PHR up to date so please take it with you whenever you take your child to see a health professional.

There are other sections that parents are asked to complete, including the Parents’ Evaluation of Developmental Status (PEDS). These important questions should be completed before your child’s developmental checks at six months, 12 months, 18 months, 2.5 years to 3.5 years, and four to five years. The answers to these questions can help identify any developmental or milestone problems with your child, and assists health profession in understanding your child better.

If you lose your child’s PHR, or you have recently moved to Queensland and don’t have one, you can get one from your nearest maternity hospital.

Child Health Information: Your guide to the first 12months

The Child Health Information booklet, inserted in the cover pocket of the PHR, provides parents and carers with useful information about the first year of a child’s life.
The booklet includes topics such as:

  • what to expect and tips for helping your baby at each age stage,
  • keeping your baby safe (including safe sleeping)
  • Developmental milestones
  • what to do if your baby gets sick,
  • caring for your baby’s teeth and ears,
  • breastfeeding, formula feeding and introducing solids.
