If you have received correspondence from Children’s Health Queensland (CHQ) regarding a patient not known, please follow the protocol outlined below to ensure the accuracy and integrity of patient records.

Recommended actions

  1. CHECK
    Please check if the correspondence is intended for you as a new patient referral, another provider in your clinic or under a different name. Please reallocate to the correct clinician internally if intended for your clinic.
  2. DO NOT file the correspondence if not intended for you or your clinic
    Please refrain from adding the correspondence to your clinical records if the patient is not known to your practice, and not being referred.
  3. Notify CHQ without delay (email: CHQ_HIMS@health.qld.gov.au)
    Prompt notification enables CHQ to investigate and rectify any inaccuracies in our records, thereby safeguarding patient safety and confidentiality.
  4. Delete the correspondence after handing back
    To preserve patient privacy, you should delete the correspondence from your system once CHQ has been notified.

When contacting CHQ, please provide the following details:

  • Your name and practice details
  • The patient’s name and date of birth as stated in the correspondence
  • Any reference number or identifiers included in the letter
  • A brief statement indicating the patient is not known to your practice
