Appointment Coordination Service
The Queensland Children’s Hospital Appointment Coordination Service aims to improve the experience of regional, rural and remote families with children and young people who have multiple appointments with different specialists at the hospital. A registered nurse works with eligible families and clinicians to coordinate a child or young person’s appointments and clinic visits to minimise the number of times a family has to travel to the hospital for their child’s outpatient care.
Who can access this service?
The service is available to families who meet all the following criteria:
- with a child or young person who attends at least 5 appointments a year with multiple specialists (including allied health) at Queensland Children’s Hospital
- who live at least 50km from the Queensland Children’s Hospital
- who are not already supported by another coordination service—such as Nurse Navigators or Connected Care.
A formal referral from a health professional is required for this service. Referrals are accepted from all health care professionals.
